Dear Parents/Guardians,

Please use this page as a resource guide to address your health concerns and locate common forms that are required by the school district. If you have any questions, need assistance with acquiring a primary care provider and/or health insurance for your child, or are having difficulty getting the required immunizations please do not hesitate to contact me by phone at 215-400-7950 option 2 or by email.

Immunization Requirements
State law requires all children entering school meet specific immunization requirements.

 

Immunizations

Does your child have a health condition that we need to know about?

As the school nurse at George W. Childs Elementary School, I would like to assist your child to have a successful school year, please let me know about your child’s health condition by completing a S-865 form each school year.

Required School Medical Forms

Any medication (prescription and over-the-counter medicines) or treatment necessary during the school day requires that the below form be completed by your child’s medical provider each school year, as well as when a change is made to a medication or treatment.

A physical exam is required for all children on original entry to Kindergarten, 1st grade, and 6th grade as well as upon entry to school for any students new to The School District of Philadelphia.

Middle school students wishing to participate in sports are required to have their medical provider complete the multi-page PIAA Physical Exam Report.

A dental exam is required for all children in 1st grade, 3rd grade, and 7th grade, as well as all any student new to The School District of Philadelphia.

Our school is in partnership with OHIP – Oral Health Impact Project for dental screenings in school. It is not required that your child participate in the program. If you would like to have your child participate in the dental screening program at school a consent form will need to be completed and signed by the parent/guardian yearly and returned to the school nurse. You may also complete the consent form via text, see below.

Mandated Health Screenings

Per The Pennsylvania Public School Code specific health screenings are to be performed in each Pennsylvania school based on grade level. Please keep in mind that these are considered screenings and are not a substitution for regular checkups with your child’s medical provider. School nurses perform the following health screenings:

·   Growth Screening: Height and Weight → All grades K-12

·   Hearing Screening → Grades K, 1, 2, 3, 7, 11

·   Scoliosis Screening (curvature of the back) →  Grade 6 & 7

·   Vision Screening → All grades K-12

The School District of Philadelphia has partnered with various health care providers to provide services for children to reach their optimal health. Our school is in partnership with WillsEye Hospital for their Wills Eye Vision Screening Program for Children.
